To Refer a Patient

  • Fax a referral to 734-232-1224
  • Use Careport (All Scripts) use of University of Michigan Health – Inpatient Rehab Unit

Required Documents

  • PM&R Consult note
  • Therapy evaluations and notes within the last 48 hours
  • Meds/labs/vitals 
  • Medication list, lab results and vital signs
  • RN notes for the last 48 hours
  • Any consult notes and most recent spell out 

Qualifications for Inpatient Rehabilitation

  • Medically require hospitalization following a traumatic injury, stroke, organ transplant, amputation, traumatic brain injury, cancer, spinal cord injury or other major disabling event
  • Require and be able to tolerate intense therapies for three hours a day, five days a week
  • Be medically stable
  • Require 24-hour nursing care
  • Require daily supervision of a Physical Medicine and Rehabilitation Physician to manage acute medical needs
