At Trinity Health Michigan, we want to meet the needs of people in our communities. We do this by working with trusted community partners every three years to conduct a community health needs assessment (CHNA) by surveying residents to hear about their needs and the conditions that affect their health and well-being.
In Muskegon County, Trinity Health Muskegon, Public Health – Muskegon County, Hackley Community Care Center, United Way and Access Health work together to conduct the CHNA survey. In Oceana County, Trinity Health Shelby serves as the lead organization for the CHNA survey.
Survey results from residents help us and our partner organizations see what needs exist, identify issues that impact health, and guide us to create a plan to improve health and well-being of people who need it most.
Between now and August 15, Trinity Health Muskegon and Shelby are asking for your feedback by completing this brief anonymous survey.
