The heart of our community: A conversation with community health workers

Community health workers (CHWs) are at the heart of Trinity Health’s mission, serving as a compassionate, healing presence within the Michigan communities they serve. Their work is rooted in trust, empathy, and a deep commitment to improving the lives of others.
Hear from three incredible CHWs serving Ann Arbor, Livingston, and surrounding communities — Mallory Hughes, Amy Bonner, and Eleni Grams. Together, they share what inspires them, what they wish more people understood about their roles, and what people can expect when working with a CHW.
Q: In your own words, how would you describe the role of a CHW?
Hughes: A community health worker is an individual who works with underserved populations to help connect them with resources within their community and provides education and advocacy to help them overcome social barriers that may hinder their health.
Q: What inspired you to become a CHW?
Hughes: I grew up in a single-income household. I had four older siblings, and I saw firsthand the barriers my family faced and how they impacted us. I am passionate about helping vulnerable populations overcome the same barriers my family experienced. Those experiences growing up inspired me to become a CHW and give back to my community.
Q: What is something you wish more people knew about the role of a CHW?
Bonner: I wish more people understood that community health workers are more than just a link to resources. We are educators, advocates, and neighbors who ensure that care truly reaches the community. We work to empower and not just assist, and our work goes beyond clinical care. We are trusted bridges and not just “helpers.”
Q: What can someone expect when working with a CHW for the first time?
Grams: Program participants can expect to undergo a thorough assessment of their social needs, as well as learn about and connect with local resources close to home. We know that asking for help can be difficult. Participants should know we are here for them, and that they will be treated with dignity and respect.
Q: How do you collaborate with partners and other organizations to strengthen your impact within the community?
Grams: When we make referrals to local community resources, we want to ensure our program participants will have success in meeting their social needs. We regularly connect with our community partners through informational events and meetings. This helps us stay up-to-date on changes within their programs, as well as maintain close relationships with organizations that may be in position to help the people we serve.
