Embedded Pharmacist
Trinity Health IHA Medical Group ambulatory care clinical pharmacist services work with primary care providers as a part of the patient-centered medical home (PCMH). Through in-office and telephone appointments, they can help patients achieve optimal control of chronic conditions, such as diabetes, high blood pressure or high cholesterol.
Clinical pharmacists provide comprehensive medication review services including, adjusting current medications to ensure maximum safety and efficacy, ongoing monitoring and disease state education. They also work with patients to set dietary and lifestyle goals to improve the management of various conditions.
A clinical pharmacist will:
- Individualize your medication regimen and/or adjust your medication doses to best manage your condition.
- Review your medications, including why they are prescribed, checking for drug-drug interactions, monitoring your treatment for maximum safety and efficacy.
- Offer suggestions on ways to lower your prescription costs.
- Review and monitor your blood sugar or blood pressure readings to identify trends and causes of elevations.
- Answer any questions you may have about your condition or medications.
Regular visits with a pharmacist, help you to achieve health goals for your chronic conditions and prevent long-term complications. Most patients see improvements in their blood sugar, blood pressure, and/or cholesterol while working together. Your clinical pharmacist is a board-certified pharmacotherapy specialist with training specific to your condition(s).
About once a month, the clinical pharmacist will meet with you in the physician’s office or speak with you over the telephone to check in, track your progress towards your goals, and discuss how your medication(s) are working. They work as part of a team with your physician and are located right in your primary care office!
