THRIVE/ACCP (Advanced Care Coordination Program) is a bridging housecalls program that services the geriatric population (65 and older). THRIVE works to coordinate care of those who have difficulty getting out of their home due to decreased function or worsening health. Our team of providers have expertise in geriatric medicine, nursing, pharmacy and work to help manage care within Trinity Health Grand Rapids and our community.
In Office Geriatric Primary Care
Trinity Health Medical Group's Comprehensive Geriatric Primary Care practice provides specialized healthcare for older adults. Services include annual wellness visits, chronic disease management, cognitive assessments, extended appointment times, and care coordination across providers and care settings. Our team is dedicated to helping patients maintain their health, independence, and quality of life as their needs change.
