THRIVE/ACCP (Advanced Care Coordination Program) is a bridging housecalls program that services the geriatric population (65 and older). THRIVE works to coordinate care of those who have difficulty getting out of their home due to decreased function or worsening health. Our team of providers have expertise in geriatric medicine, nursing, pharmacy and work to help manage care within Trinity Health Grand Rapids and our community.

Trinity Health Medical Group Geriatrics. Our geriatrics team is dedicated to providing personalized, high-quality care for older adults, with a focus on supporting patients and their families through every stage of aging. We take a comprehensive, team-based approach—offering annual wellness visits, chronic care management, cognitive assessments, and extended appointment times to ensure each patient receives thoughtful, individualized care. We also help patients set and achieve health goals while coordinating care across multiple providers and settings. Our goal is to support independence, enhance quality of life, and connect patients and caregivers to the resources they need.

THRIVE is a referral-based program so we strongly suggest speaking with your primary care physician (PCP) to determine if we may be a good fit for you.
Coming Late Summer 2026:
Trinity Health Medical Group Geriatrics will expand services at this location to include in‑office primary care for seniors, delivering coordinated, compassionate care tailored to older adults. Call 616-685-3330 to learn more.
