Share Your Voice in the Kent County Community Health Needs Assessment

Every three years, Trinity Health Grand Rapids, in partnership with the Kent County Health Department and Kent County's four other nonprofit health systems, conducts a Community Health Needs Assessment (CHNA). This survey is available in multiple languages and takes about 15 minutes to complete.

Survey results from residents help us identify the challenges and concerns in our communities, and they also help guide decisions about where resources and services are needed most. 

Between now and Sept. 7, Trinity Health Grand Rapids invites all community members to share their feedback by completing this brief anonymous survey. Your feedback can help shape a healthier future in Kent County and ensure that every voice is represented.
