Grand Rapids Community Impact Grant Program

Overview
Trinity Health Grand Rapids invites community organizations and social care providers to apply for funding to address health care gaps and barriers. The Community Impact Grants program directs hospital funds to local organizations serving vulnerable populations. This funding aims to promote optimal health by addressing social needs, connecting social and clinical care, and reducing health inequities.
Eligibility
Non-profit organizations operating in—and serving the residents of—Kent County, Michigan are eligible to apply. Grant-funded programming must align with Trinity Health's Mission, Vision and Values, and must address at least one of the top 5 priorities of the most recent Kent County Community Health Needs Assessment (CHNA):
- Housing
- Health Insurance
- Access to Medical Care
- Access to Healthy Food
- Mental Health
Review Process and Awards
Applications are reviewed and approved by the Trinity Health Grand Rapids Community Health & Well-Being (CHWB) Staff, the Community Impact Grants Review Committee, and the CHWB Steering Council. Community Impact Grants are awarded for up to $40,000 each, and no program may receive funding more than three consecutive years. Grants of $10,000 or less are awarded if funds are available after principal grants are approved.
Timeline and Process
Grants are awarded on a calendar year cycle.
The application process for 2026 grants will launch on June 1, 2025 at 8 a.m. A virtual information session is being offered on Tuesday June 10th at 1:30 p.m.
Letters of Intent are due by 5 p.m. on June 30, 2025 and must be submitted through the Trinity Health Online Grant Management System. Letters of Intent will be reviewed and invitations to submit full applications will be issued by July 31, 2025.
