To be successful, Lifestyle Medicine involves a partnership and commitment to work with your team to make key changes in the following areas to:

Shared Medical Appointments
Join a small group of patients, led by a physician who is board-certified through the American Board of Lifestyle Medicine, to explore how lifestyle changes can assist with improving and possibly reversing your chronic condition.

- Led by a provider who is certified through the American Board of Lifestyle Medicine
- Join a small group of patients to explore how lifestyle changes can assist with improving and possibly reversing your chronic condition
- Unique setting provides patients with a community of support of opportunity to share their health concerns with others facing similar challenges
- Leverage the expertise of the provider and the experience of other patients to set realistic and achievable goals
- Return every 1-2 months to discuss progress, challenges, and set new goals
- Opportunity to speak privately with the physician if needed.
Here's what you can expect during your shared appointment:
- Available in-person and virtually on Zoom
- 90-minute to two-hour visits ensure plenty of time to address your individual questions and concerns
- Visits limited to no more than 12 patients
- Post-visit follow-up from the care team to help navigate goals between visits
- Billed as a normal medical visit and subject to copay

Individual Consultation
Consult one-on-one with a board-certified Lifestyle Medicine provider to improve — and potentially reverse — chronic conditions such as diabetes, high blood pressure, high cholesterol, obesity, and more.
What to Expect:
- One-on-one consultations with a certified Lifestyle Medicine provider
- A focus on improving and possibly reversing chronic conditions
- A goal to decrease your need for medications
- Support from Lifestyle Medicine trained nurses, nutritionists, and the care team
- Virtual or in-person visits, with follow-ups as needed over 3–9 months
- Primary care continues with your current provider throughout
- Return to regular care with your primary care physician after the consult process
This is a short-term consult service designed to empower you on your journey to better health.
Shared Medical Appointments
Join a Shared Medical Appointment (SMA) to meet with a small group of patients led by a Lifestyle Medicine provider. Learn together, support each other, and take practical steps to reach your health goals.
What to Expect:
- 1.5 to 2-hour group visits every 1–2 months
- Up to 12 participants per group
- Offered in-person or virtually
- Expert guidance from a Lifestyle Medicine-certified physician or nurse practitioner
- Supportive, collaborative environment with patients facing similar challenges
- Private time with your provider if needed
- Billed like a standard medical visit (subject to co-pay)
