What is Lifestyle Medicine?

Helping you get to the root cause of chronic disease through evidence-based care.

Lifestyle Medicine is a medical specialty that uses evidence-based lifestyle changes to treat and possibly reverse chronic disease. Our goal is to help you thrive, not just manage symptoms.

Eighty percent of chronic diseases are driven by lifestyle choices — but that also means they can often be treated or improved with the right support.

Healthy Living Makes a Big Difference

People who followed five key healthy habits—eating well, being active, maintaining a healthy weight, not smoking, and drinking alcohol in moderation or not at all—have:

  • 74% lower risk of dying from any cause
  • 82% lower risk of dying from heart disease
  • 65% lower risk of dying from cancer
Researchers estimate that over 60% of early deaths could be prevented if everyone adopted these habits.

Why It Works:

  • Backed by strong clinical evidence for preventing, treating, and even reversing chronic disease
  • Proven to improve outcomes for conditions like diabetes, heart disease, and high blood pressure
  • Uses sustainable lifestyle changes instead of quick fixes or temporary solutions
  • Rooted in science, guided by medical professionals, and tailored to your needs
  • Empowers you with tools, education, and ongoing support for long-term health

Individual Consultation

Consult one-on-one with a board-certified Lifestyle Medicine provider to improve — and potentially reverse — chronic conditions such as diabetes, high blood pressure, high cholesterol, obesity, and more.

What to Expect:

  • One-on-one consultations with a certified Lifestyle Medicine provider
  • A focus on improving and possibly reversing chronic conditions
  • A goal to decrease your need for medications
  • Support from Lifestyle Medicine trained nurses, nutritionists, and the care team
  • Virtual or in-person visits, with follow-ups as needed over 3–9 months
  • Primary care continues with your current provider throughout
  • Return to regular care with your primary care physician after the consult process

This is a short-term consult service designed to empower you on your journey to better health.

Shared Medical Appointments

Join a Shared Medical Appointment (SMA) to meet with a small group of patients led by a Lifestyle Medicine provider. Learn together, support each other, and take practical steps to reach your health goals.

What to Expect:

  • 1.5 to 2-hour group visits every 1–2 months
  • Up to 12 participants per group
  • Offered in-person or virtually
  • Expert guidance from a Lifestyle Medicine-certified physician or nurse practitioner
  • Supportive, collaborative environment with patients facing similar challenges
  • Private time with your provider if needed
  • Billed like a standard medical visit (subject to co-pay)

These visits combine medical care with group education and support — a powerful combo!
