About this event

Trinity Health's Medicare Diabetes Prevention Program (MDPP) is a year-long program for older adults with Medicare who are ready to make healthy lifestyle changes and prevent type 2 diabetes.

Learn how to be more active, manage stress and lose weight while learning techniques that will help you stay positive and motivated. This nationally recognized, yearlong program begins with 16 weekly sessions, followed by monthly maintenance sessions to provide ongoing support.

Program Eligibility
The Medicare Diabetes Prevention Program is open to individuals who meet the following requirements:
- Medicare Part B Coverage (Fee for Service) and
- Have not received MDPP services previously
- Have a BMI greater than 25 (or greater than 23 if identified as Asian)
- No history of Type 1 or Type 2 diabetes and no diagnosis of End Stage Renal Disease
- Results of a blood test within the past year showing
    - Hemoglobin A1C: 5.7%–6.4% OR
    - Fasting plasma glucose:110–125 mg/dL OR
    - Two-hour plasma glucose (after a 75 gm glucose load): 140–199 mg/dL
